The \device\00000058 operation is done using the logical block address 0x0. It was found that the logical block address 0x0 does not work in the usual manner.

It appears that when the logical block address 0x0 was used to access the object for the first time after the operation was retried, it was not the same logical block address that was used the second time.

After this operation, the disk 3 was not able to read a disk address from the logical block address 0x0. This is a rather odd behavior to have.
